JAMESTOWN – The Chautauqua County Department of Health and Human Services, Division of Public Health closed its Jamestown clinic on 110 E. Fourth St. on September 1, 2015.
The County opted to close the clinic due to decreased patient volumes, increased operating costs and the expansion of the Federally Qualified Health Center, The Chautauqua Center, into Southern Chautauqua County.
“We are so fortunate to have a Federally Qualified Health Center in Chautauqua County,” said Christine Schuyler, Director of Health and Human Services. “It just makes good financial and patient care sense to support the expansion of the Chautauqua Center. There is so much overlap in the services and the Chautauqua Center is poised to provide more holistic care to the patient that the Public Health Clinics do not provide.”
The Chautauqua Center’s Jamestown office is located in the former Public Health Clinic space at 110 E. Fourth St. in Jamestown.
The Division of Public Health will continue providing services at its clinic on 7 N. Erie St. in Mayville. Services will be provided at the Division of Public Health clinic on 319 Central Ave. in Dunkirk through the end of the year.
Patients that need assistance locating a new provider; scheduling appointments or that wish to have records transferred to another provider should call the Division of Public Health at 1-866-604-6789.
